Be committed to making positive changes

Attend 15 minute weekly case management sessions

Communicate regularly with a Coordinator

Continually work to achieve goals
Eligibility Requirements

Must reside in the state of Maryland
-
Must be a MD Medicaid recipient or be eligible for MD Medicaid
-
Age 5 and up
-
Actively participating in therapy or be willing to attend therapy
